1 This document must be printed on four piece carbonless paper consisting of a white first page, pink second page, gold third page and a yellow fourth page with the “Notice To Permit Holder/Occupant OF The Protected Premise/Property Owner” to be printed on the back of this fourth page in order to be accepted by the Nassau County Fire Marshal’s Office. This type of paper is available at most stationery stores.
